“Exercise reduces inflammation” is the kind of health claim that gets repeated so often it stops meaning anything specific. A new network meta-analysis[1] pooling 55 randomized controlled trials and 3,516 middle-aged and older adults puts real numbers behind it — and, more usefully, tells you which type of exercise to reach for depending on what you’re trying to lower.

Not all exercise lowers the same markers

The analysis compared exercise modalities head-to-head against three specific inflammatory markers: C-reactive protein (CRP), interleukin-6 (IL-6), and tumor necrosis factor-alpha (TNF-α) — all three linked to age-related chronic disease when they stay chronically elevated.

Resistance training came out ahead for CRP specifically, producing the largest drop of any modality tested. Aerobic training took the lead for IL-6 and TNF-α. Neither modality dominated across all three markers — which is the actual finding worth remembering, since most casual exercise advice treats “cardio” and “lifting” as interchangeable for inflammation control.

Why this matters past 40

Chronic low-grade inflammation — sometimes called “inflammaging” — is one of the more consistent biological threads connecting aging to cardiovascular disease, type 2 diabetes, and frailty. The practical takeaway from this analysis isn’t “exercise more,” it’s “exercise on purpose”: if a lab test shows elevated CRP, resistance training has the stronger evidence behind it; if IL-6 or TNF-α are the concern, aerobic work has the edge.

The authors’ own framing backs this up — they describe “customized structured exercise” as the effective intervention, not exercise in general. A mixed training week that includes both resistance and aerobic work covers the bases on all three markers, which is the simplest way to apply this without needing your own lab panel to pick a lane.
