Tension and stress
The scalp is covered in muscle, and it holds tension the same way your shoulders or jaw do. Chronic stress, teeth clenching, and even tension headaches can radiate into scalp tenderness that has nothing to do with your hair or skin directly.
Hairstyle-related traction
Tight ponytails, braids, or extensions pull on hair follicles repeatedly, which can cause a genuinely inflamed, tender scalp over time — sometimes called traction alopecia when it’s severe enough to affect hair growth. Loosening up the style is often the first thing dermatologists suggest.
Skin conditions
Psoriasis, seborrheic dermatitis, and folliculitis (inflamed hair follicles, sometimes from sweat, product buildup, or ingrown hairs) can all cause a sore, itchy, or burning scalp. These usually come with visible flaking, redness, or small bumps alongside the pain.
Nerve-related pain
Occipital neuralgia — irritation of nerves at the base of the skull — can cause sharp, shooting scalp pain that’s easy to mistake for a skin issue when it’s actually neurological.
When to see someone
Mild, occasional tenderness tied to a tight hairstyle or a stressful week usually resolves on its own. Persistent pain, visible rash or flaking, patches of hair loss, or pain that’s sharp and localized is worth a dermatologist visit — scalp pain has enough different causes that self-diagnosing from a symptom list alone isn’t reliable.
